Support at Home has an Assistive Technology and Home Modifications scheme. Funding, assessment and provider arrangements need to be confirmed through the program; a website enquiry does not establish access to funding.
Speak with your aged care provider first
Ask which assessment and approval steps apply to your circumstances. Confirm the approved scope and whether funding has actually been allocated before arranging paid work. Do not assume an ongoing service budget can simply be redirected to building work.
Keep the prescribed modification clear
The program guidance sets out professional prescription requirements for home modifications. Your provider and occupational therapist should explain what information is needed. The building scope then needs to reflect the agreed work rather than a general wish list.
Ask who will commission and pay for the work
Clarify whether the provider is arranging the contractor, how quotes are compared and whether you have a contribution. These responsibilities should be clear before a contract is signed or products are ordered.
Prepare a practical site enquiry
Tell us the address, proposed rooms or access changes, timeframe and whether the provider is already involved. You do not need to include medical history or a full report in the initial website form. Detailed document sharing can be arranged separately if needed.
What to include in your enquiry
Tell us the property address, the activity or room that is difficult, your timeframe and whether a professional recommendation is available. There is no need to send medical records through the form.
Project suitability, the actual contracting provider, applicable credentials and any required approvals must be confirmed before building work is agreed.
